PATIENT PORTAL

  • The Family Clinic offers our patients secure internet access to their medical information on line, so you may view your personal health record at any time from any  place with internet access. You will be given a username and password, and will receive periodic updates through your personal email address on file.

​INSURANCE

  •  Please bring your insurance card with you to your first and subsequent  visits.  We must have this to file your insurance claims.  Absent your insurance card, you have to pay your entire bill.
  • Knowing your co-pay and deductible along with your wellness benefits when coming to your visit is very helpful to our clinic.


PHYSICALS

  • Physicals for sports, employment, immigration, insurance applications and other administrative reasons are frequently not covered by insurance and shall be paid for at the time of service. We encourage everyone to keep a yearly visit with your provider for health maintenance reasons such as cancer screening, blood pressure checks, blood sugar/cholesterol checks, and healthy lifestyle counseling. It is your responsibility to arrange this visit so that we can help you stay healthy.

​PRESCRIPTIONS

  • Please bring all your prescription medications in their original container to each visit.
  •  Refills should be obtained during your visit to the office. Call-in prescriptions are discouraged and may not be allowed in some instances.
  • A fee may be charged for prescriptions not associated with an office visit. The decision to call in a prescription always lies with the medical provider.
