  • The Family Clinic offers our patients secure internet access to their medical information online, so you may view your personal health record at any time from any  place with internet access. You will be given a username and password, and will receive periodic updates through your personal email address on file.
    • Request appointments (for non-urgent issues)
    • Request prescription refills
    • View medical records and lab results (PHR)
    • View current and past statements
    • Send non-urgent messages to clinical and clerical staff
    • Receive health maintenance reminders

  • Please bring all your prescription medications in their original container to each visit. It is vitally important for your safety and good health, that we maintain an up to date detailed list of you medications.  
  • Refills should be obtained during your visit to the office. Call-in prescriptions are discouraged and may not be allowed in some instances.
  • A fee may be charged for prescriptions not associated with an office visit. The decision to call in a prescription always lies with the medical provider.


  • Administrative Physicals:  Services such as sports clearance, employment assessments, immigration, insurance applications, Basic Med flight exams, disability assessments  and other administrative services  are generally  not covered by insurance and shall be paid for at the time of service.
  • Wellness visit:  We encourage everyone to schedule  a yearly visit with your provider for health maintenance reasons such as cancer screening(breast, colon, prostate, lung), blood pressure checks, blood sugar/cholesterol checks, and healthy lifestyle counseling.

  • Please bring your insurance card with you to your first and EVERY visit.  We must have this to file your insurance claims.  Absent insurance  verification, you have to pay your entire bill.
  • Knowing your co-pay and deductible along with your wellness benefits when coming to your visit is  helpful to your avoiding making overpayments.
  • We are not participating providers in Medicare United Healthcare and Humana Advantage plans.
